The Government of the Northwest Territories is asking NWT residents to complete the 2025 Patient Experience Questionnaire.
The purpose of the questionnaires is to gather service users’ feedback about their experiences accessing and receiving healthcare services in the last 12 months in the Northwest Territories.
In-patient services are those healthcare services that require you to be admitted to hospital or an acute care facility in the NWT for at least one overnight stay.
Outpatient healthcare services are those that do not require an overnight stay in hospital (such as primary community care, surgical day care, rehabilitation, eye clinic, etc.)
Completing a paper version of the questionnaire available through local health centres in NWT communities.
NWT residents who have accessed healthcare services in the last 12 months are encouraged to participate by completing the confidential questionnaire before November 10, 2025.
Interpretation services are available to service users by request at health care centres and through the GNWT’s active offer.
Next Steps
Results from the questionnaire are used to measure the effectiveness of the health care system in meeting the needs of patients accessing services in the territory and will help the Health and Social Services system improve and monitor progress over time.
The Patient Experience Questionnaire is a part of the health and social services system’s monitoring and reporting activities, where patient experience is considered a key dimension of quality care.
